A. Scheduling

Registration—Use form A-1 (DOC, XLS, PDF).  Once all flags have been lifted, student will be able to register on ARROW.

Drops/Adds—Performed on ARROW before the published deadlines.  (See University Calendar).  Reference the “Dropping and Adding Courses” section in the current catalog.Overloads—Use form A-2 (DOC, XLS, PDF).  A student taking more than 18 hours in fall or spring semesters or 6 hours in a summer term must apply for an overload.  Requires the approval of the Department Head and the Dean.

Course Substitution—Use form A-3 (DOC, XLS, PDF).  Requires the approval of the Department Head and the Dean.

Waiver of Prerequisites—Use for A-4 (DOC, XLS, PDF).  Requires the approval of the instructor and the Department Head.  Original form should be sent to the student’s Dean’s office.

Late Registration or Course Addition—Use form A-5 (DOC, XLS, PDF).  Student should seek the instructor’s written permission for each course.  This documentation should be attached to the form.

Late Drop and Resignation—Student should seek the instructor’s written permission to drop a course after the deadline.  This documentation should be attached to the drop slip, which should be sent then to the department head and subsequently the dean.  Reference the “Course Drop and Resignation Policy” section in the current catalog.

Switching Sections—Use form A-6 (DOC, XLS, PDF).  Requires the approval of the terminal instructor and the student’s Department Head.  Original form should be sent to the student’s dean’s office with the Drop/Add slip.

Catalog Change—Use form A-7 (DOC, XLS, PDF).  Requires the approval of the Department Head and the Dean.

Rescheduling Final Exam—Use form A-8 (DOC, XLS, PDF).  Requires approval of the course’s instructor and the student’s Dean.

Permission to take Courses at another University—Use form A-9 (DOC, XLS, PDF).  Requires the approval of the Department Head and the Dean.

Remove Developmental Requirements—Use form A-12 (DOC, XLS, PDF).  Requires the approval of the Department Head and the Dean.

B. Graduation

Graduation Checkout—Performed each semester after a student has attained senior status (90 hours).  Send student to Dean’s office at the beginning of the student’s last semester for final checkout.

Graduation In Absentia—Use form A-10 (DOC, XLS, PDF).  Requires the approval of the Dean.

C.  Appeals

Grade Appeal—Reference the current catalog section “Procedure for Appealing a Grade” and the Student Policy Paper section “Appealing a Grade”.

Suspension Appeal—Reference the current catalog section “Probation, Suspension, and Readmission Regulations”.

Academic Renewal, Academic Bankruptcy, Academic Clemency, or Academic Forgiveness—Reference the current catalog section “Academic Renewal”.

Conflict Resolution—Email College Advisor.

Change of Advisor— Use form A-11 (DOC, XLS, PDF). must be completed and submitted to the Dean’s Office so that student’s record can be updated in the system.
