Cost of Attendance Increase Request (link to pdf)
Federal regulations allow the cost of a computer to be added to your cost of attendance budget. This increase cannot exceed $1500 and can be made only once during the time you attend ULM. The purchase of the computer must take place during the academic year in which the budget revision is requested.
Dependency Override Form (link to pdf)
This form allows you to request special consideration of your dependency status for financial aid purposes for the 2012-2013 academic year. Although you do not meet the requirements to be considered an independent student, you believe your family circumstances warrant further evaluation.
2012-2013 Satisfactory Academic Progress Appeal Information and Forms
Students who have been notified by the Office of Financial Aid that they are ineligible to receive financial aid due to not making satisfactory academic progress are required to submit a Satisfactory Academic Progress Appeal form. For the appeal to be reviewed, the completed form must be returned with an explanatory letter of your circumstances and any supporting documents of those circumstances. Your appeal must be approved in order to receive financial aid at the University of Louisiana at Monroe in the future.
