Proof of Immunization or Waiver Process
All ULM students (including those completely online) are required to show proof of
all required immunizations OR have an exemption recorded. Follow these steps to submit your completed immunization record or exemption form:
Step 1: Scan or take a picture of your completed immunization record or exemption form.
- The Proof of Immunization Compliance form can be found HERE. Students should take the form to their physician for completion or they can upload
their state immunization record.
- Students wishing to declare an exemption/waiver can find the form HERE.
Step 2: Find your username and password.
- Visit https://webapps.ulm.edu/myulm/myulm_accountlookup
- Enter your Campus-Wide ID
- Make note of your username
- Initial password is your birth month, birth day, and year, followed by a dash and
the last 4 digits of your CWID. Ex: 090307-1234
Step 3: Upload the scan or photo of your immunization record or exemption form.
- Visit https://ulm.medicatconnect.com/home.aspx
- Enter your username and password
- Click “Uploads” tab.
- Click the "Select File" button next to the "Immunization Exemption Form or Proof of
Immunization Compliance Form" and locate the file on your computer or smart phone.
- Click the "Upload" button. The document will be listed under "Documents already on
file" as a confirmation that the document was successfully uploaded.
Students who do not submit the required immunization information or exemption form
will have an immunization hold preventing them for registering for classes. Students needing assistance can email admissions@ulm.edu.
