The following must be completed before the first day of school next fall. Except where specific contact information is provided below, questions can be directed to the Office of Student and Professional Affairs, 318-342-3800.
Current ULM students may skip this step. Accepted applicants who do not already attend ULM must apply for general admission to the university online at http://www.ulm.edu/admissions/apply.html. Choose the undergraduate transfer application (not the graduate application) and indicate pre-pharmacy as your major. Questions about this portion of the general admission process should be directed to 1-800-372-5127. This step should be completed as soon as possible.
The College of Pharmacy requires vaccination and proof of immunity in the form of serology for the following infections: measles (rubeola), mumps, rubella, hepatitis B virus, and varicella virus. Vaccination for tetanus and diphtheria is also required. A vaccination record for the College of Pharmacy to be completed by a healthcare provider will be given to you during fall orientation. This date will be announced in the summer.
Students not attending ULM at the time of admission to pharmacy school must also submit a Proof of Immunization Compliance form to the ULM Office of Admissions as part of the general admission process for the university. This form is available at http://www.ulm.edu/immunization/.
Students must maintain a health insurance policy during enrollment in the College of Pharmacy and must provide proof of current insurance coverage at orientation and prior to each semester. Coverage as part of a parent’s or spouse’s plan meets this requirement. Any student may apply for the student health insurance that is offered by the University through the Office of International Students. Contact Sami Owens (318-342-1885; saowens@ulm.edu) for details. Students failing to provide proof of health insurance may have their offer of admission rescinded or be suspended from the program until such proof is available.
All students are required to be certified in Basic Life Support (BLS) for the Healthcare Provider prior to entering the College of Pharmacy. This certification should include infants, children, and electronic defibrillators. Students without previous certification will have the opportunity to obtain certification during the ULM College of Pharmacy Boot Camp, which takes place the week before the fall semester.
